We are glad to see that you have come to God seeking forgiveness. The Bible tells us, If we confess our sins, he is faithful and righteous to forgive us the sins, and to cleanse us from all unrighteousness. (1 John 1:9 WEB).
Forgiveness is a wonderful gift from God, made possible through the sacrifice of Jesus Christ on the cross. It is by the name of Jesus Christ that we have access to God. And in no other is there salvation, for neither is there any other name under heaven, that is given among men, through which we must be saved! (Acts 4:12 WEB).

Dear Heavenly Father,
We come before You in the name of Jesus Christ, lifting up this person who has asked for Your forgiveness. Lord, You are merciful and gracious, slow to anger, and abounding in steadfast love and faithfulness. You do not treat us as our sins deserve, nor repay us according to our iniquities. (Psalm 103:8, 10 WEB).
We confess that we all have sinned and fall short of the glory of God (Romans 3:23 WEB), and we thank You for the precious blood of Jesus, which cleanses us from all sin. We ask that You would forgive the sins of this person and cleanse them from all unrighteousness.
In Jesus' name, we pray. Amen.

That if you will confess with your mouth that Jesus is Lord, and believe in your heart that God raised him from the dead, you will be saved. For with the heart, one believes resulting in righteousness; and with the mouth confession is made resulting in salvation (Romans 10:9-10 WEB).
