Firstly, let us address the essence of our prayers. It is crucial to remember that it is in the name of Jesus that we have access to God. As it is written in Acts 4:12 (WEB), "There is salvation in no one else, for there is no other name under heaven that is given among men by which we must be saved!" So, we will pray to God the Father in Jesus' name.
Now, let's bring your requests before the Lord:

Dear Heavenly Father,
We come before You in the precious name of Jesus, seeking Your mercy and intervention in ###'s life. Lord, You promise that if we confess our sins, You are faithful and righteous to forgive us our sins and to cleanse us from all unrighteousness (1 John 1:9). So, we ask for ###'s restoration, that You would cleanse and forgive, and grant a fresh start.
