We are truly blessed to join you in prayer and seek God's will for your situation. It is commendable that you have turned from your past life of sin and have become a new creation in Christ. As it is written in 2 Corinthians 5:17 WEB, "Therefore if anyone is in Christ, he is a new creation. The old things have passed away. Behold, all things have become new."
However, we must address a few things before we pray. First, it is important to acknowledge that the breakup occurred because of your past actions, and it is not surprising that she is avoiding you, especially if she was genuinely hurt. It is also crucial to understand that her current feelings and actions may be a result of the pain and betrayal she experienced. The Bible Finding a spouse is incredibly important and is not something to be rushed or forced.
Proverbs 4:23 WEB says, "Keep your heart with all diligence, for out of it is the wellspring of life." It should be our prayer and desire for this woman to guard her heart as well as to heal from the past. She needs to know that God is her protector and that He has a plan and purpose for her life.
As for reconciliation, it is essential to remember that true forgiveness and restoration come from God. While we can pray for a second chance it is more important to pray for forgiveness and for God's will to be done. Psalm 37:4-5 WEB says, "Delight yourself also in Yahweh, and he will give you the desires of your heart. Commit your way to Yahweh. Trust also in him, and he will do this:"
Regarding your desire for a Christ-centered family, it is wonderful to see your heart's longing for a godly marriage. However, remember that marriage is a lifelong commitment between a man and a woman, and it should not be entered into lightly. It should be a commitment between two believers who are equally yoked and seeking to glorify God in their union.
2 Corinthians 6:14 WEB says, "Don’t be unequally yoked with unbelievers, for what fellowship have righteousness and iniquity? Or what fellowship has light with darkness?"

"Father, we come before You, lifting up this man who has turned from his past and has become a new creation in Christ. We pray for this woman who has been hurt and is guarding her heart that You would heal her and comfort her and give her strength and guidance. We ask that You would cause them both to seek You and Your will in their lives. We also pray that You would soften their hearts towards each other only if it is Your will, and that You would guide them in Your truth. Grant them both discernment, wisdom, and patience as they navigate this situation. We ask this all in Jesus Name."

Help and strengthen me God to always respect and obey You. Bless me to trust You with all my heart, acknowledge You in all my ways, and lean not to my own understanding.
