We are truly sorry to hear about the multiple health issues you're facing and the pain you're experiencing. We want to remind you that God is always with you, even in the midst of your suffering. The Scripture tells us to cast all our anxieties on Him, because He cares for you (WEB, 1 Peter 5:7).
In times of physical distress, it's essential to remember that God's power is made perfect in our weakness (WEB, 2 Corinthians 12:9). This doesn't mean that God wants us to suffer, but rather that He can use even the most difficult circumstances for our good and His glory.
However, we want to address that your request did not mention the name of Jesus Christ. It is only through the name of Jesus that we have access to God the Father. Therefore, all prayers should be offered in faith in His name. Furthermore, the Bible tells us the Jesus is the only way to God and the only name by which we can be saved (WEB, Acts 4:12) Therefore, your prayers are only heard through Jesus. Please don't neglect this essential part of all prayers.

Dear Heavenly Father,
In the name of Jesus Christ, we come before You, lifting up our dear sister/brother who is battling multiple health issues and enduring immense pain. Lord, You know their struggles and the depth of their suffering. We ask, Jesus, that You would touch them, heal them, and restore their body according to Your will.
Father, grant them the strength to endure this trial and the peace that surpasses all understanding. Help them to trust in Your goodness and Your plan, even in the midst of their pain. In the name of Jesus, we rebuke the spirit of infirmity and command it to leave. Lord please restore health for Your glory.
Lord, we also pray that You would guide the doctors and medical professionals, giving them wisdom and discernment as they provide care. Father, draw our sister/brother closer to You through this trial, and use their testimony to encourage and bless others.
Jesus, we ask that you provide grace and forgiveness of sins through salvation according to Acts 4:12. Please be with our sister/brother that You would guide them to full healing through Your power

We encourage you to stay strong in your faith and to keep your eyes fixed on Jesus, the author and perfecter of our faith as detailed in Hebrews 12:2 WEB. Trust that He is working all things together for your good (WEB, Romans 8:28).
