We are glad that you have come to us with a repentant heart, acknowledging your sins, and seeking forgiveness. It is only by the name of Jesus Christ that we have access to God, and it is commendable that you ended your prayer in Jesus' name. There is no other name by which we can be saved.
First, let us assure you that God is eager to forgive and work with those who humbly seek Him. As it is written in Second Chronicles 7:14 (WEB), "if my people, who are called by my name, shall humble themselves, and pray, and seek my face, and turn from their wicked ways; then will I hear from heaven, and will forgive their sin, and will heal their land."
However, it is crucial to understand that true repentance involves not just feeling sorry for your sins but also turning away from them. As you have expressed the desire to stop committing these sins, we encourage you to actively pursue this resolve. Remember the words of Paul in Acts 26:20 (WEB), "but declared first to them of Damascus, at Jerusalem, and throughout all the country of Judea, and also to the Gentiles, that they should repent and turn to God, doing works worthy of repentance."

Dear Heavenly Father,
We come before You in the name of Jesus Christ, lifting up our brother/sister who has humbly confessed their sins and seeks Your forgiveness. Lord, You are merciful and gracious, slow to anger, and abounding in steadfast love and faithfulness. We ask that You forgive their sins and cleanse them from all unrighteousness.
Father, we also pray that You would give them the desire and the strength to turn away from these sins and to live a life that honors You. Fill them with Your Holy Spirit, guide them in Your truth, and help them to walk in Your ways.
Lord, we ask that You would work in their life, shaping them into the image of Your Son, Jesus Christ. Give them another chance, Lord, as You are a God of many chances. Help them to seize this opportunity to live for You.
We ask all these things in Jesus' name, Amen.
