We are truly saddened to hear about the situation you are facing, and we want to address it with care and biblical principles. Firstly, we must address the elephant in the room, the woman who is trying to become a man. This is against what the Bible teaches, as it is written in Deuteronomy 22:5 WEB A woman shall not wear men's clothing, nor shall a man put on women's clothing, for whoever does these things is an abomination to the LORD your God. Therefore, we must rebuke his behavior because it is unacceptable and encouraging sin. It would be best if this individual got biblical counseling for personal identity to find out who they are in Jesus Christ and not in what they think the world wants them to be. And Romans 12:2 - Do not be conformed to this world, but be transformed by the renewing of your mind, so that you may prove what is the good, well-pleasing, and perfect will of God.
Regarding his relationship, the Bible teaches against homosexual relationships in Leviticus 18:22 You shall not lie with mankind, as with womankind. It is abomination. While we understand that it is important to love and pray for those who are lost, we must not condone sin. We also want to clarify that only Jesus Christ provides us with access to the throne of God. It would be best if you mentioned that your trust is in Jesus Christ. I John 5:13 These things have I written unto you that believe on the name of the Son of God; that ye may know that ye have eternal life, and that ye may believe on the name of the Son of God.
Now, let us address the concerns about the mother and her son. It is important to seek godly counsel and involve the authorities when there is a threat of physical harm. Proverbs 3:5-6 reminds us to trust in the Lord with all our heart and not lean on our understanding. In all your ways acknowledge him and he will make your paths straight.
Regarding the fight with his girlfriend and the possibility of hiding medical damages because of being called names by the son, we will pray that any infirmed decision made by the mother will be granted as grace because of a decision made to fear rather than faith. Faith is supposed to be used rather than fear. Unfortunately, the mother is in a state of fear due to the son's relationship and its consequences.

We come before you in the name of Jesus Christ to lift up this mother and her son. Lord, we pray for their safety and protection. We ask the Holy Spirit to convict the son of his wrongdoing and guide him towards repentance. Help him to make changes in his life. We pray that you may have mercy on the one that is confused in identity but also trying to harm those around them rather than loving them as we ought. Persecution of Christians will occur but not by the hand of those in a sinful relationship. So please have mercy for both of them. We pray for the mother's health and peace of mind. Sufficient is thy grace.
We ask Jesus Christ, our intercessor, to intercede on their behalf and bring about godly spouses for both of them to look for and enter into marriage with. Lord, we pray for the authorities to act justly and swiftly so that the services of the hospital may be granted in the name of Jesus Christ. We ask all of these things in accordance with thy will. Please place the mother and her son in positions to receive thy blessing and thy grace. Let the convictions of wrongdoing be made evident to the both of them and most importantly your servant.
