Firstly, it is wonderful that you acknowledged the name of Jesus in your request. It is only by His name that we can come before the Father. Let us remember the words of Jesus in John 14:6 WEB: "Jesus said to him, I am the way, the truth, and the life. No one comes to the Father, except through me."
Regarding your grandmother's feelings, it is important to remember that God has not given us a spirit of fear, but of power, love, and self-control. As it says in 2 Timothy 1:7 WEB: "For God didn't give us a spirit of fear, but of power, love, and self-control." Therefore, let us put our trust in Him.

Dear Heavenly Father,
We come before You today, lifting up ### to Your throne of grace. We thank You for her life and the love she shares with her family. Lord, we ask that You would comfort her heart and calm her spirit, casting out all anxiety and fear.
In Jesus' name, we rebuke any spirit of fear that may be troubling ###. We ask that You would fill her with Your perfect love, which drives out all fear.
We pray for Your protection over ###, guarding her from all harm and danger. We claim Your promise in Psalm 91:11 WEB: "For he will put his angels in charge of you, to guard you in all your ways."
We also pray for Your healing power to be upon ###, blessing her with good health and a long life, according to Your will.
