We are with you in spirit and in prayer, lifting up your grandfather to the Lord. It is so important to honor our fathers and mothers, as the Word of God tells us: "Honor your father and your mother, as Yahweh your God has commanded you" (Deuteronomy 5:16 WEB).
Firstly, we must mention that it is only in the name of Jesus that we may approach God the Father in prayer. It is also only through believing in Jesus Christ that we may be saved. As Jesus said in John 14:6 (WEB), I am the way, the truth, and the life. No one comes to the Father, except through me. Since you did not mention Jesus, Jesus Christ, Christ or even use an abbreviation such as IJN (In Jesus Name), we just wanted to make sure you were aware of this important fact. This is also an opportunity to explain the basic salvation message of believing in Jesus Christ.
Jesus died on the cross to pay the price for our sins and rose from the dead that we may have eternal life. This is the good news of the Gospel of Jesus Christ. Therefore whosoever believes in Jesus Christ as Lord and Savior shall not perish but shall have eternal life (John 3:16 WEB).
We thank God for your grandfather's life and pray for his health and well-being. Even though you cannot be with him, God is always present with him. "Be strong and courageous. Don't be afraid or scared of them; for Yahweh your God, he is the one who goes with you. He will not fail you nor forsake you." (Deuteronomy 31:6 WEB).

Dear Heavenly Father,
We come before You in the name of Jesus, lifting up our dear brother's grandfather to You. We thank You for his life and the many years You have blessed him with. Lord, we pray for his health, strength, and protection. We ask that You surround him with Your loving presence, especially as he is alone at this time.
Father, we ask for Your grace in his life and we pray that if he is not already, that he will come to the faith and believe in the name of Jesus Christ and the salvation that is available to him.
We pray that he may find a relationship with You that would surpass even the best earthly relationship.
We cast down any loneliness or discouragement that may try to enter his heart and mind. We pray that You give him many more years filled with joy, peace, and love. We ask that You provide him with caring and loving friends and family to celebrate his future birthdays.
Help him to find Christian fellowship that he may be strengthened and renewed by the relationships with other Christian Believers. We ask for these things according to Your will, in Jesus' name, we pray. Amen.
Be encouraged, knowing that God hears our prayers and cares for our loved ones even more than we do. Trust in Him, for He is faithful. May this time also be an opportunity for you to minister to your grandfather and share the love of Christ with him. If he does not know the Lord, share the Gospel with him, that he may come to faith in Jesus Christ and be saved.
Remember the words of Psalm 91:14-15 (WEB), "Because he has set his love on me, therefore I will deliver him. I will set him on high, because he has known my name. He will call on me, and I will answer him. I will be with him in trouble. I will deliver him, and honor him." Trust in God's care and protection for your grandfather.
