We are truly saddened to hear that you feel disconnected from God, ###, but we are here to help guide you back to Him. Firstly, it is important to remember that there is no sin too great for God to forgive. The Bible tells us, If we confess our sins, he is faithful and righteous to forgive us the sins, and to cleanse us from all unrighteousness -1 John 1:9 (WEB). Even when we are unfaithful, He remains faithful, for He cannot deny Himself -2 Timothy 2:13 (WEB).
It is commendable that you acknowledge your sin and disobedience. We have all sinned, and fallen short of the glory of God -Romans 3:23 (WEB). Sin separates us from God, but He has provided a way back to Him through His Son, Jesus Christ. For the wages of sin is death, but the free gift of God is eternal life in Christ Jesus our Lord -Romans 6:23 (WEB).
We must believe in Jesus to be saved. There is salvation in no one else, for there is no other name under heaven given among men, whereby we must be saved -Acts 4:12 (WEB). We are saved by Grace of God and not by works. Not by works of righteousness which we have done, but according to his mercy he saved us, by the washing of regeneration and renewing of the Holy Ghost -Titus 3:5 (WEB).

Dear Heavenly Father,
We come before You today to lift up ###, who feels lost and separated from You. Lord, we know that You desire for none to perish, but for all to come to repentance. We confess that ### has sinned and disobeyed Your commands, and we ask for Your forgiveness on their behalf. Wash away their sins, Lord, and make them whiter than snow.
We pray that You would soften ###'s heart and draw them close to You once again. Help them to see that a relationship with You is not about feelings, but about faith. Give them the faith to believe in Jesus Christ as Lord and Savior. We ask that You would give them assurance of their salvation and the power of the Holy Spirit to walk in obedience to Your will.
