Regarding your grandmother's words, it's crucial to discern whether they were spoken out of concern or out of a critical spirit. The Bible tells us to test every spirit to see whether they are from God (1 John 4:1 WEB). While it's possible that God could use your grandmother to speak truth into your life, it's also possible that her words were hurtful and not inspired by God. Proverbs 15:4 WEB says, "A gentle tongue is a tree of life, but deceit in it crushes the spirit." We should pray for wisdom to know the difference.
It's also important to note that while God can speak through others, He primarily speaks to us through His Word and His Spirit. If there are areas in your life that need change, God will convict you personally. Therefore, be wary of accepting condemnation from others without seeking God's confirmation. Also, even if you forgot to do a chore or accidentally got your grandmother wet, these actions do not warrant harsh words or accusations of mental instability. It may be helpful to discuss this with your grandmother. However, make sure it is done in a God honoring way with a humble attitude as you explain your feelings and possibly apologize for any pain you caused.
Concerning your mental health, there's no shame in seeking help from a therapist or counselor. God often uses professionals to bring healing and restoration. However, ensure that any professional help you seek aligns with biblical principles and does not replace your dependence on God.
Also, if you are struggling with sin in your life, you can be assured that God will forgive you and help you repent. If we confess our sins to him, he is faithful and just to forgive us our sins, and to cleanse us from all unrighteousness. (1 John 1:9 WEB) God is always ready to forgive and restore us when we come to Him with a repentant heart.
