We understand that you're going through a tough time and want to help. Firstly, we notice that while you mention needing Jesus, you did not actually write his name when requesting prayer. It is only through the name of Jesus Christ that we can approach God the Father. Therefore, we pray this in Jesus' name.

Dear Heavenly Father,
We come before You today in the name of Jesus Christ, lifting up our fellow brother/sister in need. They have acknowledged their need for Your intervention in Jesus' name and their desire to stop certain things which will need your strength and guidance. We have all sinned and gone astray from Your glory, and we humbly ask for Your forgiveness and mercy as your children. Please, Lord, help them overcome the things they cannot stop in their strength, and guide them back to a life pleasing to You according to your will.
We know that You are faithful and just to forgive us our sins and to cleanse us from all unrighteousness. We claim the promise in 1 Corinthians 10:13 WEB: "No temptation has taken you except what is common to man. God is faithful, who will not allow you to be tempted above what you are able, but will with the temptation also make the way of escape, that you may be able to endure it." Help both us and the requester to also see the escape path when temptation comes.
Provide them the strength needed to resist the sins and path they are currently stuck on. Guide our beloved brother/sister on the only way out and the only path to eternal life, which is through explicit faith in the way of Jesus Christ and by God's grace continue in that faith. They understand that they cannot change this sin or addiction based upon on their own strength. Let them continue to look at You when they find no solution to these problems, so we all can say "No sin is greater then the grace that You give. Help them to avoid the Devil's traps, and show them the way to true freedom and to believe in the atoning sacrifice of Jesus Christ.
We are all called to be a light to the world and use our testimonies and character. How lovely it is to give hope and encouragement to the broken and oppressed and to give hope to our neighbor. If we truly love our neighbor, our actions will reflect that love through our assistance. Aid us to give assistance and guidance to everyone we can. We are all called to be Your hands and feet on this Earth as we await the resurrection. Galatians 6:1-2 clearly states "Brothers, even if a man is caught in some fault, you who are spiritual must restore such a one in a spirit of gentleness; looking to yourself so that you also aren’t tempted. Bear one another’s burdens, and so fulfill the law of Christ."
Please, Father, guide them to a local church in Jesus' name and that recognizes that the word of God is the only path to salvation. Encourage them to be a part of a Bible-believing fellowship where God's word and the preaching of the cross are the core teachings (Philippians 1:27 and 1 Corinthians 1:18-31 and 1 Corinthians 9:16).
