We understand your heartache and concern for your loved one, and we stand with you in prayer. Let's first address the issue of addiction and codependency, as well as the spiritual warfare you're engaged in.
Firstly, it's crucial to understand that our battle is not against flesh and blood, but against the rulers, against the authorities, against the world rulers of this darkness, against the spiritual forces of wickedness in the heavenly places. Therefore, take up the whole armor of God, that you may be able to withstand in the evil day, and having done all, to stand. Ephesians 6:12-13
Regarding addiction, it is written:
Be sober and self-controlled. Be watchful. Your adversary the devil, walks around like a roaring lion, seeking whom he may devour. 1 Peter 5:8
And for codependency, remember that we are all individual temples of the Holy Spirit, and we should not be unequally yoked with unbelievers, separating yourselves from idolatry. You should not be tangled up with the affairs of the world, but be separate from those who are not aligned with the teachings of Jesus and live a holy life.

Now, let us pray together:
Dear Heavenly Father,
We come before You in the name of Jesus Christ, lifting up our loved one who is battling addiction and codependency. Lord, we know that this is not just a physical battle, but a spiritual one. We ask that You break the chains of addiction and set ### free from his demons, in Jesus' name.
Father, we pray for his job and his children. Protect them from the schemes of the enemy and let no weapon prosper against them. Remove anyone who is not of You out of his path, so that he may walk in Your will and live a life that honors You. Exodus 23:23
Lord, we also pray that You would bring all generational curses to an end. We claim the promise that You will show mercy to a thousand generations of those who love You and keep Your commandments. Therefore, let him and his children serve You, their family protected from the evil one. Deuteronomy 5:10
Father, we ask that You would give him and his family strength and courage to serve You. Let them be a light in this world, drawing others to You. We thank You for Your faithfulness and mercy. We trust in You to bring restoration and healing to this family. Isaiah 5:20
Let them be in the world, but not of the world; let them be separate from idolatry, being used as instruments of righteousness, and let them be sanctified and holy.
Father, guide us all to a closer relationship with You. Let us not rely on our own understanding, but trust in You with all our hearts. Also, let us all acknowledge You in all our ways so that You can direct our paths. We pray all things in Jesus' name, as we know there is no other name by which we can be saved. Proverbs 3:5-6 and Acts 4:12
We also pray that anyone who reads this prayer will come to a saving knowledge of Jesus Christ, and we pray for Christians who believe to draw closer to God.
Finally, we ask that You would give us all discernment to recognize the spiritual forces at work in our lives and the lives of our loved ones. Help us to stand firm against the enemy's schemes and to live in the victory that Jesus won for us on the cross. Galatians 2:20

Lastly, we want to encourage you to continue praying and trusting in the Lord. Let God have all your anxieties, worries, and fears in prayer and supplication, with thanksgiving, requesting the peace of God to guard your hearts and minds. Philippians 4:6-7
Remember, it is only by the name of Jesus that we have access to God. It is only through Jesus that we can be saved. Therefore, continue to pray in His name, trusting in His promises and believing in Him to be the way, the truth, and the life.
