We understand that you're seeking forgiveness for a past mistake, and it's wonderful that you're turning to God in your time of need. The Bible tells us, "If we confess our sins, he is faithful and righteous to forgive us the sins, and to cleanse us from all unrighteousness" (1 John 1:9 WEB).
However, we noticed that you didn't mention the name of Jesus Christ in your prayer. It's important to remember that it is only through Jesus that we have access to God. As it is written, "For there is one God, and one mediator between God and men, the man Christ Jesus" (1 Timothy 2:5 WEB). And in the book of Acts, it is declared, "There is salvation in none other, for neither is there any other name under heaven, that is given among men, by which we must be saved!" (Acts 4:12 WEB).
Let us pray together:
Heavenly Father, we come before You in the name of Jesus Christ, seeking forgiveness for the mistake committed twelve years ago. Lord Jesus, You said that if we confess our sins, You are faithful and just to forgive us and cleanse us from all unrighteousness. We confess this sin before You and ask for Your forgiveness. Help us, Lord, to never repeat this mistake again. Guide our steps and strengthen us to walk in Your ways. In Jesus' name, we pray. Amen.
Remember, God is always willing to forgive those who genuinely seek Him. As Psalm 103:12 WEB tells us, "As far as the east is from the west, so far has he removed our transgressions from us." Go in peace, knowing that your sins are forgiven, and strive to live a life pleasing to God.

Protect me from all the plans of my enemies and the plans of the enemy of my soul.
