We understand that you are seeking prayers from your church pastor to be remembered by your family after 40 days. While we will pray for you, we want to ensure that your focus is on the eternal rather than the temporal.
The Bible tells us in Colossians 3:2 WEB, "Set your mind on the things that are above, not on the things that are on the earth." Our ultimate goal should be to glorify God and have an eternal impact rather than just being remembered on earth. We encourage you to use this time to reflect on your relationship with God and your family with Psalm 90:12 WEB saying, "So teach us to number our days, that we may gain a heart of wisdom."
In Jesus name, God wants our hearts to be seeking after him and his righteousness. If Jesus is not at the center of your prayer request then nothing else matters, no other alignment is of any consequence.
We also want to remind you of the importance of praying in Jesus' name. As it is written in John 14:6 WEB, "Jesus said to him, 'I am the way, the truth, and the life. No one comes to the Father except through me.'" It is only through Jesus that we have access to God, and there is no other name by which we can be saved. Acts 4:12 WEB

Dear Heavenly Father,
We come before You in the name of Jesus, lifting up this individual who desires to be remembered by their family. We pray that their heart is turned towards You, seeking Your will and Your glory. Help them to use this time to grow closer to You and to reflect Your love to their family. We ask that You grant them wisdom and discernment as they navigate this season, and that their family would see Jesus in them.
In Jesus' name, we pray. Amen.
We encourage you to share your faith with your family, and to live in a way that points them to Jesus. As the body of Christ and as family members, we are called to love and serve one another, with the goal of mutual edification and encouragement. This will have a much more lasting impact, through the Holy Spirit, than just being remembered.
