insurance claim form

Pray that God will grant me a good Dr in auto immune dept to look at my blood test report and MRI reports and grant me sufficient and correct advices, I forget to bring the insurance claim form of the critical illness but I hope the Dr will let me know if it will costs money to hv them filling...
